Individualized 1:1 Supports

Our team works with you to reach your goals. This might include in person sessions, online supports, or both.​ Funding may come from the Self-Determination Program (SDP), a school settlement, or elsewhere.

Our individualized supports can be called many different things:

 

• Independent Living Skills (ILS) (SDP code 320)

• Supported Living Supports (SLS) (SDP code 320)

• Tailored Day Services (TDS) (SDP code 331)

• Community Integration (SDP code 331)

• Educational training (SDP code 334)

• Communication supports (various SDP codes)

• Educational buyout

• Transition support
